Consent for Services & Office Policies

Please review the policy below. When you are finished reviewing, fill in your name, the names of the minor child(ren) receiving services, and click the checkbox to indicate you have read and understand the policy.

I, the undersigned, do hereby authorize and give consent to Aardvark Family Services to provide services to the minor child(ren), listed below, for the purpose of joint requested or court-ordered, Supervised Visitation Time. I authorize Aardvark Family Services to investigate, review, and disseminate all information provided to Aardvark Family Services by the undersigned or others, relating to allegations of abuse and/or neglect involving the child(ren). By signing this agreement, undersigned acknowledges that Aardvark Family Services’ primary interest in this matter is the safety and well-being of the child(ren) and Aardvark Family Services will not align itself with any party, parent or guardian related to this matter although Aardvark Family Services staff will act to prevent harm to the child(ren), which may put them temporarily in conflict with an allegedly abusing or harmful party, parent or guardian. Aardvark Family Services determines that such alignment is in furtherance of the safety and well-being of the child(ren). Supervisors reserve the right to terminate a visitation at any point for any reason so long as it is in the best interest of the child(ren).

Aardvark Family Services retains sole authority and discretion relating to all decisions regarding its involvement in this matter, including but not limited to whether and/or when it will agree to provide services, the nature and scope of any services it provides, and when its involvement in this matter will cease. Aardvark Family Services will make every effort to fully comply with the Court’s orders regarding time/access.

THIS AGENCY: It is within the scope of this agency to offer assistance for families and divorced/separated parents when child(ren) are involved, when there are allegations of child(ren) abuse whether substantiated or unsubstantiated, or when child(ren) may not be receiving the rights to which they are entitled. As child(ren) advocates, we are child(ren) focused and will gear our sessions toward the best interests of the child(ren). Unreported allegations of abuse will be referred to the State of Tennessee as mandated by law.

DISCLOSURE OF CONFIDENTIAL INFORMATION: While it is not the practice of Aardvark Family Services to disclose confidential information to others, we will make confidential information available to be used for the purpose(s) of current legal proceedings or for legal proceedings that may arise in connection with this/these child(ren) and this/these current issue(s). Thus, all supervision services and communications are deemed non-confidential.
